1. Is Fuel to Nourish an in-network provider with my plan (please refer to NPI 1679399166)

2.  If not in network, what are my out-of-network benefits and how can I submit for reimbursement? 

3. Does my plan cover the following CPT codes of 97802 (medical nutrition therapy assessment) and 97803 (medical nutrition therapy follow up) for outpatient nutrition counseling?

4. Do I need a referral or prior authorization?

5. Do I have a co-pay for each visit? If so, how much is my co-pay?

6. Does my deductible apply? If so, how much is my deductible?

7. Do I have any session limits or maximum amount of sessions I can be seen for? 

8. Do I have a maximum benefit limit for the year?

9. Do I have co-insurance? If so, what percentage will I be responsible for?​​

 

If you have a different plan or carrier, you may be able to use out of network benefits for potential reimbursement. We can provide you with superbills to submit to your insurance for out of network coverage for nutrition sessions.
